RED BANK – VNA Health Group has received a grant for $10,000 from OceanFirst Foundation to support the Institute for Children and Family Health (ICFH).
The grant will help with the capital campaign for planning and launching the institute.
The Institute for Children and Family Health will provide opportunities for clinical excellence, innovation, and growth, while enhancing access and patient experience and improving outcomes for patients, their families, and caregivers. ICFH will be a resource of connected programs and services that will address patients’ needs; staff will assist them in navigating through a complicated health and social service system, all while maintaining them in a network of care at home and in the community.
The programs will include prenatal and postpartum care, home visitation for first-time or at-risk mothers and children, special child health services, early intervention services for children with specials needs, specialized pediatric care in the home, primary and preventative health services, school-based health services, pediatric hospice care, and integrated care coordination.
